If you’re ready to get started click the button below to begin your enrollment. Enrollment Fee will be processed after registration is completed and one fee per household. Our current enrollment promo, you will be charged 200$ on registration and receive half that as a credit toward your first visit saving you 100$! If registering an additional household member click ‘add a patient’ on the left of the screen before finalizing your enrollment. After you’ve completed the enrollment form you’ll receive an email/or text message or phone call to schedule an initial physical exam and counseling session. Next you’ll be invited to download our App providing easy access to your Electronic Medical Record**. Within our new system, you will be able to monitor/track your progress, medications, interact with your medical team, make appointments, pay your statements, and much more! Additionally, you’ll receive yet another email with important questionnaires addressing various aspects of your health**. Feel free to complete them as your time allows and before the initial physical exam. There is a 20$ fee to process credit card refunds.

***Due to limited appointment availability, there is a 60$ no show/no call fee billed for new patient physicals cancelled within 24 hours and it will be automatically charged to the card on file.
